What is the Subscription Program?
The PCAA Subscription Program is an annual membership that covers your household's out-of-pocket ambulance costs for emergency and medically necessary transports within our service area — above and beyond what your insurance pays.
Subscribers may be billed for a portion of the balance after insurance.
How to Enroll
- 1Download or print the subscription form below.
- 2Complete all fields including household member names.
- 3Mail the completed form with your check or money order to our address.
- 4You will receive a confirmation card by mail.
